• Indicator edits are now logged. Every save on Key Respondents, Data Source, Result Summary, Result Detail, Maturity Level, or attachments records a change log entry with the list of fields that actually changed — a real audit trail, not just “last edited by”.
• Every activity log entry now shows the actor's organisation alongside their name, both on the assessment Activity tab and in the new dimension-level panel. The organisation is captured at the moment of the write, so historical entries stay readable even if a user later moves.
• Word round-trip imports now produce a single clean “imported changes to D3 from a Word document · 4 indicators updated” row per affected dimension, instead of one row per touched indicator.
• Activity feeds now paginate — first 10 entries shown, click “Show older entries (N more)” to reveal the next 10. Applies to both the assessment Activity tab and the dimension-level Change log.

• Guards built in throughout — mismatched-assessment uploads are rejected outright, out-of-date documents surface a “server has newer edits” warning, and applied imports emit a proper entry on the change log.
• PDF upload is coming in a follow-up. For now, upload the .docx file directly.
